Big data cognitive healthcare intelligence from CognitiveScale

CognitiveScale has released the Guided Care suite of cognitive healthcare insights. The company says Guided Care, which is powered by cognitive technology, is designed to help patients, care managers and population health executives effectively treat and manage chronic conditions including cancer, diabetes and heart disease.

Further, it claims, the Guided Care suite can help reduce emergency room visits annually by more than five percent and reduce hospital use rates up to 30 percent over five years, and represents a convergence of artificial intelligence, natural language processing and feedback-driven dynamic learning software. CognitiveScale explains the Guided Care suite works on massive amounts of structured and unstructured data to generate personalized insights from “dark” data, described as data that is not machine readable, not easily accessible or not leveraged.

CognitiveScale reports the Guide Care suite is comprised of three major offerings:

  • Guided Concierge—reach and engage high-risk patients with one or more chronic conditions through secure, highly personalized insights to help steer them toward better and more effective care compliance;
  • Guided Advisor—empower care managers with relevant and contextual insights about patients and population risk gleaned from clinical, social, environmental and socio-economic data, enabling them to intervene in a timely manner to impact care outcomes and reduce avoidable hospitalizations; and
  • Guided Explorer—inform population health executives with insights about their patient populations, enabling them to proactively understand population risk, manage quality of care, reduce costs and deliver better outcomes.
